Consumer Health Data Privacy Policy

This Consumer Health Data Privacy Policy explains how Ignite Phyzio & Sports Performance (“Ignite,” “we”) collects, uses and shares “consumer health data” as defined by Washington’s My Health My Data Act and similar state laws, such as those in Nevada and Connecticut. It supplements our general Privacy Policy. It does not apply to patient treatment records, which are protected under our patient privacy practices and applicable medical privacy laws.

Consumer health data we collect

  • Information about an injury, surgery or recovery that you choose to share, such as the type of injury, where you are in your recovery, or how many weeks you are after surgery
  • Health-related questions or messages you send us through forms, email, text or social media
  • Information that identifies you as seeking health care, such as a request for an evaluation, consultation or appointment
  • Registration for health-related education sessions or programs, such as Office Hours or our online ACL programs

We do not collect precise geolocation, biometric data or genetic data through our website.

Sources

We collect consumer health data directly from you when you fill out a form, register for an event, purchase a program, or contact us. We may receive limited information from a parent or guardian completing a form for an athlete, or from a clinician or coach who refers you to us with your permission.

Why we collect and use it

  • To provide the service you requested, such as registering you for a session, answering your question or scheduling an evaluation
  • To tailor educational content to where you are in your recovery
  • To improve our education programs and services
  • To comply with the law and protect the security of our Services

We collect and use consumer health data only with your consent or as necessary to provide a product or service you requested. We do not use it for targeted advertising.

Who we share it with

We share consumer health data only as needed to provide what you requested, or with your separate consent. We share it with these service providers, which process it on our behalf under contract:

  • HubSpot, Inc. (customer relationship management, forms and email)
  • Squarespace, Inc. (website hosting and forms)
  • Zoom Communications, Inc. (video sessions)
  • Scheduling and payment providers, when you book or purchase a service

We do not have affiliates that receive consumer health data. We do not sell consumer health data, and we will not do so without a separate, signed authorization from you as required by law.

Your rights

  • Confirm whether we collect, share or sell your consumer health data, and access that data, including a list of the third parties and affiliates we have shared it with
  • Withdraw your consent to our collection and sharing of your consumer health data
  • Have your consumer health data deleted, including from our service providers’ systems

To exercise these rights, email info@ignitephyzio.com or call 714-335-3182. We will verify your request and respond within 45 days, which we may extend by 45 more days when reasonably necessary, with notice to you. If we decline your request, you may appeal by emailing us with the subject line “Health Data Appeal.” We will respond within 45 days. If your appeal is denied, you may contact the Washington State Attorney General at atg.wa.gov/file-complaint or your own state attorney general.

We will not discriminate against you for exercising any of these rights.
